New Patient Paperwork
Upon scheduling an appointment with Dr. Jazi here you will have access to the new patient forms. If you are unable to fill out the forms online you can fill them during your in person visit with Dr. Jazi.

Dr. Jazi is in network with Medicare. If you have Medicare please make sure to send his office (or bring your Medicare and ID card to your first appointment). Dr. Jazi’s practice will submit the claim and will charge your card on file, if you have any copay. The amount of co-pay is usually known only after claim submission is complete. If you don’t have Medicare please see Dr. Jazi’s fees below. Dr. Jazi can send superbills to you.

Psychotherapy Sessions
Lots of conditions can be treated only by psychotherapy, Intensive Short Term Dynamic Psychotherapy is what Dr. Jazi practices to help your mental health conditions. The focus is on your emotions.
